Frequently Asked Questions About Spravato

Comprehensive Answers for Your Guidance

  • What is Spravato?

Spravato® (esketamine) is an FDA-approved nasal spray for adults with treatment-resistant depression and major depressive disorder (MDD). It may be used alone or with an oral antidepressant.

Unlike traditional antidepressants that primarily affect serotonin, Spravato works on the brain’s glutamate system (NMDA receptors) and may help improve symptoms more rapidly in some patients.

  • Who is a good candidate for Spravato?

Spravato may be appropriate for adults whose depression has not improved after at least two antidepressants or who require more rapid symptom relief. A comprehensive psychiatric evaluation is required to determine eligibility.

  • Is Spravato the same as ketamine?

Spravato contains esketamine, a form of ketamine that is FDA-approved specifically for depression and administered under strict medical guidelines. It differs from IV ketamine infusions offered at some clinics.

  • How is Spravato administered?

Spravato is self-administered as a nasal spray in our Ashland, KY office under supervision. Blood pressure is checked before treatment, and patients remain for approximately two hours afterward for monitoring.

You are required to arrange transportation home. Driving is not permitted until the next day after a full night’s sleep.

  • How often is treatment given?

Treatment typically begins twice weekly for four weeks. After the initial phase, sessions are spaced weekly or every other week based on response and symptom stability.

  • How quickly does Spravato work?

Some patients notice improvement within the first few treatments, while others may require several weeks. Individual response varies.

  • What are the most common side effects?

Common side effects include dissociation (feeling disconnected), dizziness, nausea, drowsiness, increased blood pressure, headache, and a temporary unpleasant taste.

These effects usually occur shortly after dosing and resolve the same day. Spravato is not commonly associated with weight gain or sexual side effects.

  • Are there additional risks?

Spravato can temporarily impair attention, judgment, and reaction time on the day of treatment.

There is potential risk of misuse, particularly in individuals with a history of substance use disorder. In rare cases, worsening depression or suicidal thoughts may occur, especially early in treatment.

  • How should I prepare?

Arrange a responsible adult to drive you home.

Avoid eating for 2 hours before treatment and drinking liquids for 30 minutes beforehand to reduce nausea risk.

Plan to remain in the office for about two hours after each session.

  • Who should not receive Spravato?

Spravato is not approved for individuals under 18.

It should not be used in patients with:

    • Aneurysmal vascular disease
    • Arteriovenous malformations
    • History of intracerebral hemorrhage
    • Allergy to esketamine or ketamine

It is not recommended during pregnancy. A full medical evaluation is required before treatment.

  • Will insurance cover Spravato?

Many commercial plans, Medicare, and Kentucky Medicaid cover Spravato for approved indications. Prior authorization is usually required.

Our team verifies coverage and manages the authorization process before treatment begins.

  • Do I need a referral?

A referral is not required. We coordinate with your current providers when appropriate.

  • How do I get started?

Schedule a consultation to determine whether Spravato treatment in Ashland, KY is appropriate for you.
